Options to refer a Patient

  1. Email referral to the Patient direct  ie to their personal email
  2. Email referral to Clinpath Client Services  clientservices@clinpath.com.au
  3. Scan/email referral to Clinpath  clientservices@clinpath.com.au

  5. Send print referral via your Clinpath courier.

REQUIRED DETAILS ON REFERRAL

All pathology referrals require the following:

  • Patient’s full name and date of birth
  • Patient’s Medicare card number
  • Patient’s full address
  • Tests Requested
  • Clinical notes as required
  • Full referring doctor details, including address and provider number
  • All copy doctor details

If you send the request directly to Clinpath, we will contact the patient to arrange the booking, so please include patient contact details.

  3. If you do not have a request form, we can arrange one for you and your negative result will be sent by SMS.
  4. We recommend that all COVID-19 results be discussed with your GP.
